This sounds like a good dua. Inshallah I will begin using it also. 

Perhaps you may want to consider adding the Arabic script as well. Sometimes people find that easier to read than English transliteration (myself included).

But I am glad you added the translation and meaning of the terms.

May Allah reward you and guide us all.
